*RN Support Coordinator - Full-time, $5,000.00 Sign-on bonus**Basic Job Duties:*Conducts assessments, reassessments, and supports coordination for A+D MI Choice Waiver. Provides and coordinates case management services including the necessary documentation regarding client care according to the client plan of treatment. Successfully communicates with the office staff, support coordinators physicians and other agencies providing services to a client.*Requirements:*Registered Nurse eligible to practice in the State of Michigan. Graduate of an accredited School of Nursing. A thorough understanding of confidentiality and HIPAA principles is required.
