Our facility located in Lorain is easily accessible from Rt 2 and Rt 6. The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient. We are committed to providing care so every resident can reach peak independence and functionality. Our dedicated therapy team has a proven track record of getting results! The therapy gym is spacious and equipped with all the tools that a therapist needs to provide the most effective treatment interventions.**Responsibilities:**- Conduct assessments and develop personalized treatment plans for patients.- Administer therapeutic exercises and modalities to improve patients' physical well-being.- Collaborate with healthcare professionals to ensure comprehensive patient care.- Educate patients and their families on treatment plans and home exercises.- Maintain accurate and timely documentation of patient progress and outcomes.- Stay updated on the latest advancements in physical therapy techniques and technologies.**Experience:**- Degree in Physical Therapy from an accredited program.- State licensure as a Physical Therapist or eligibility for licensure.- Experience in hospital settings, trigger point therapy, pediatrics, or home community care is advantageous.- Proficiency in medical terminology and anatomy knowledge.- Strong interpersonal skills and the ability to work effectively in a team environment.- Passion for providing high-quality patient care and improving individuals' quality of life.Join our team at Heritage Rehab where you will have the opportunity to make a meaningful impact on the lives of our patients while advancing your career as a skilled Physical Therapist.
