Job Description:

The Intake / Crisis Coordinator provides screening and intake interviews to children and their families when one child is mentally or emotionally disturbed, and who are at risk of hospitalization, or vulnerable individuals. This professional collaborates with psychiatric and allied team members in treatment planning and intervention. This clinician completes initial paperwork and coordinates case assignments. ESSENTIAL DUTIES and RESPONSIBILITIES include, but may not be limited to the following:1. Provides initial intake assessments as needed.A.Intakes and related activities are completed appropriately. B.Reports to work on time as scheduled and at the location assigned.C.The clinician demonstrates the ability to determine resource and service needs in a comprehensive manner.D.Makes case / program assignments appropriately.E.Completes CAFAS as assigned cases within required timeframes.2. Provides individual / family planning and review as related to client needs. A.May assist or develop written individual service plans, reviews, and termination summaries on the basis of client/family needs. 1.Practices Person Centered Planning principles. 2.Service planning integrates assessments to provide meaningful individual goals and objectives. 3.Service and annual reviews reflect the client's ongoing treatment services and the development of additional necessary goals and objectives. 4.Progress notes are related to goals and objectives listed in the treatment plan.B.The Clinician completes necessary documentation in a timely manner. 1.Intakes are completed in a timely and thorough manner. All assessments contain the documentation requested of the form. 2.All related forms are reviewed with family and completed as needed. 3.Progress notes are completed after each contact. 4.Service plans and reviews are completed within established timeframes.3. The intake / crisis staff conducts intakes and crisis intervention effectively.A.The clinician has knowledge of treatment interventions and has the ability to apply them effectively in therapy.B.The clinician has knowledge community resources needed and available, as shown in his or her ability to educate clients to gain insight into participation in services.C.The clinician has the ability to triage effectively.D.The service recommendations are reflective of the client's identified problem areas.4. The intake / crisis staff participates in clinical team meetings and coordinates team interventions.A.The clinician is able to develop treatment goals and objectives, which are consistent with the child's, family's and other clinical team members.B.The clinician consults with other team members when appropriate and integrates their input as appropriate.C.The clinician takes responsibility for assigned crisis coverage.5. The clinician demonstrates professional and ethical behavior.A.The clinician continues to demonstrate proficiency in his or her professional practice by attending regular case conferences, in-services, and workshops, and required trainings.B.The clinician has no substantiated recipient rights violations filed against him or her.6. Direct service time.A.The clinician maintains 85 hours of direct service time, unless job assignments given with supervisory approval require a variance to this expectation.7. The clinician participates in committees and assumes other tasks as requested.QUALIFICATIONS - To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.EDUCATION and/or EXPERIENCE - Masters degree in social work, psychology or related fields from an accredited college or university preferred. Preferred 2 years experience in working with mentally or emotionally disturbed children. LANGUAGE SKILLS - Excellent communication skills, both verbal and written. Ability to establish professional working relationships with all staff. Appropriately articulates the view of the agency when networking in the community.CREDENTIALING, LICENSURE - Social Work registration with the State of Michigan preferred. Without registration, staff will participate in supervision to monitor and validate work. Valid State of Michigan Drivers' License required.
